In accordance with Texas Health & Safety Code §181.105, this page provides our patients with information about accessing your medical records, contacting licensing authorities, and filing a complaint.

How to Request Your Medical Records

You have the right to review and obtain a copy of your health records. To request your medical records from Austin Osteopathic Medicine, please submit a request using one of the following methods:

By Patient Portal

Send Dr Lytle-Vieira a message in your patient portal requesting a copy of your health records. She will send you a medical records release form for you to complete.

Your request should include:

Your full legal name, date of birth, contact information, a description of the records you are requesting, and your preferred format (electronic or paper).

Response Time

Your records will be provided within 15 business days of receiving your written request.

Fees

You may be charged a reasonable, cost-based fee for copying and mailing your records. You will not be charged a retrieval fee.
